Re-Enrollment for the 2025–2026 School Year is Now Open!
We’re excited to kick off re-enrollment for the 2025–2026 school year! Lock in your child’s seat and stay with the Perry Street Prep community. Let’s make next year even better — starting with two easy steps!
Families should have received a notification that our re-enrollment process for the 2025–2026 school year is now open! As a reminder, Perry Street Prep has a 2-step re-enrollment process:
✅ Step 1: Intent to Return
Complete the Intent to Return Form that was emailed to you.
✅ Step 2: Submit Your Annual DCRV Application
Submit your updated District of Columbia Residency Verification (DCRV) form with all supporting documents.
📋 Additional Requirement: Health Forms:
Students entering the following grades must also submit a copy of their updated health forms:
- Pre-K3
- Kindergarten
- Grade 7
While DC law requires all students to be current with their immunizations, students in these specific grades are required to submit health documentation as part of enrollment.

🛡️ More on Immunization Requirements
District of Columbia law requires that all students attending school in DC are up to date on their immunizations to protect the health of the entire school community.
According to DC Code § 38–501 et seq., and enforced by the Office of the State Superintendent of Education (OSSE):
"No student shall be admitted by a school unless the school has certification of immunization for that student, or the student is exempted pursuant to law."
In other words, students who are not in compliance with immunization requirements may be excluded from attending school until the proper documentation is submitted.
📝 What You Need to Do:
- Submit an updated DC Universal Health Certificate
- Submit the DC Oral Health (Dental) Certificate
- Ensure your child’s immunizations are current according to the DC Health Immunization Schedule
