Re-enrollment is a three step process here at Perry Street Prep.

  1. Submit a paper copy of your Intent to Return.
    • (Deadline to submit is December 16, 2017)
  2. Complete your InfoSnap Online Registration.
    • (Deadline to submit is February 9, 2017)
  3. Submit updated Proof of Residency.
    • (In Person submittal window: April 3 - May 3, 2017)

All families who submit their Intent to Return form, by the deadline, will receive an email on December 16, 2016 regarding online registration and next steps. In addition to registering online there are several items that must be submitted onsite to complete the enrollment process each year.

Re-Enrollment Checklist 

  • Online Registration 
  • Proof of DC Residence (must submit documents in person April 3, 2017-May 3, 2017)
  • Updated DC Universal Health Certificate 
  • Updated DC Oral Health Certificate
 
The admissions office is open to receive paperwork daily from 8am - 11am and 3pm - 4pm.  On 4/5/17 and 4/26/17,  the office will be open on until 5:30pm.  Copies of all required forms are available in the office.
 

If you don't have access to a computer, there are computers available in the Admissions Office for your use. Additionally, printed copies of any necessary form can be obtained from the Admissions Office.

Missed Deadlines?  Remaining seats are available to current and new families on a first come, first serve basis after May 8, 2017.
